Who qualifies for services at Senior Friendship Health Center?

Collier County residents who are 50 years of age or older and fall within 300% of the Federal Poverty Guidelines. Individuals are required to complete an application to qualify and will be contacted to schedule an appointment to establish care. Senior Friendship Health Center participates with Medicare and Medicaid.

Please bring the following items for income verification:
  • Pay stubs (last 4 weeks of income)
  • Last Year’s Income Tax Return or Schedule C if self Employed
  • Worker’s Compensation (last 4 weeks of income)
  • Unemployment Compensation (last 4 weeks of income)
  • Disability or SSI Benefits Letter
  • Social Security Income
  • Letter of Support (notarized signature)

Is the Senior Friendship Health Center a walk in or urgent care facility?

No, the Senior Friendship Health Center is not a walk-in clinic or urgent care. We provide primary, specialty and dental care via scheduled appointments and for regular health maintenance. Patients who are experiencing acute or emergency symptoms will be referred to the nearest emergency department for treatment.

Is the Senior Friendship Health Center a free clinic?

The Senior Friendship Health Center is not a free clinic. We provide services on a sliding scale based on household income and dictated by the Collier County Department of Health. Patients are expected to pay at time of service.

Who provides the care at the Senior Friendship Health Center?
